As a past participant in the Cabin Fever Classic, or as an interested party, we would like to invite you to play in our tournament this year. Feel free to make copies of this entry form and pass them along to any of your fellow competitors you think may be interested in playing. Because of its popularity at other events we run, we have added a buffet meal after Saturday's round at this year's event. Thank you for your past support and we look forward to seeing you at this year's tournament.

    

Date:    Saturday and Sunday - February 18th and 19th, 2012

Format:    Two-man scramble. 36 holes. Gross by division.

Divisions:    Up to six divisions based on combined team handicaps.

        NO MINIMUM HANDICAP THIS YEAR!

        PROFESSIONALS WELCOME! (One per team)

        Maximum partner handicap difference     7

Entry fee:    $120.00 per player ($240.00 per team).

Entry fee includes green fees for the tournament proper, prize fund, buffet meal following play on Saturday, coffee and donuts before play each day, plus unlimited range balls before play each day.

Practice rounds:    A complimentary weekday or weekend after 11:00 am round will be extended to those entrants who can play prior to the tournament.

Prizes:    $4200.00 in prizes, based on a full field of 60 teams.

Tee times:    9:00 AM SHOTGUN BOTH DAYS

Optional side games:    Side game competitions will be available each day. All side     game payouts are for pro shop credit only.

Entries are accepted on a first come, first served basis, upon a fully paid team entry fee.

Entry deadline: Friday, Feb. 10^th, 2012 or the first 60 fully paid entries, whichever comes first. All entries received after this will be considered on a space available basis. The handicap revision that will be used for this event is February 15^th, 2012
